About this resource

This page introduces a study that followed 1,037 people born in Dunedin, New Zealand, in 1972–1973 from childhood to age 38 and examined persistent cannabis use in relation to changes in neuropsychological functioning. A distinctive feature was that cognitive functioning was measured in childhood before cannabis use began and reassessed in adulthood.

A prospective cohort can establish temporal order more readily than a study that asks participants to recall the past after an outcome. It was not, however, an experiment randomly assigning cannabis use, so associations cannot establish causation. Living environment, education, other substances, mental health, and other factors influence long-term outcomes.

Study design

Participants were interviewed at several ages, with cannabis use and dependence assessed over time. Intelligence tests and other measures completed before age 13 served as the baseline, and neuropsychological tests of memory, processing speed, executive functioning, and other domains were administered at age 38. Analyses examined relationships among persistence, age at initiation, number of dependence diagnoses, and cognitive change.

Ratings of everyday functioning from family members and acquaintances were also used, reducing reliance on laboratory scores alone. Exposure, however, was primarily self-reported, and THC concentration and amount per occasion were not measured.

Content

The study centered not on a simple comparison between users and nonusers in adulthood but on within-person change from childhood to adulthood. This partly accounts for pre-existing cognitive differences and makes it easier to examine the temporal relationship between persistent use and change.

The authors analyzed education, other drugs and alcohol, schizophrenia, and several alternative explanations. Because the study remained observational, unmeasured factors and complex bidirectional relationships still could not be excluded completely.

Results

The group showing persistent cannabis dependence at multiple assessment points had a reported decline in neuropsychological performance from childhood to age 38, with a more marked association among those whose persistent pattern began in adolescence. The results also corresponded with ratings by other people of problems involving attention and memory.

This average association does not mean every user experienced the same change. Findings were not uniform across adult initiation, discontinuation, and different frequencies, and group sizes were limited. Subsequent research has continued to debate confounding, replication, and measurement methods.

Limitations

The cohort came from one region and birth period and does not represent current diverse populations or high-THC products. Amount of use and product constituents were not chemically measured, and effects of tobacco, other substances, and social environment remain. The persistent-dependence group was a subset of the cohort, leaving uncertainty in estimates.

An observational study cannot establish whether stopping use always reverses change or at what point any change becomes irreversible. Change in cognitive-test scores cannot be simplified to a single cause or to the entirety of an individual’s ability.

Safety

Young people are in a period when the brain and foundations of daily life are developing, and this evidence argues against dismissing long-term associations with frequent, persistent use. At the same time, an individual’s test decline should not be attributed diagnostically to cannabis alone or used to define that person’s character or ability.
