About this resource
This is an independently written detailed introduction, based on the PubMed bibliography and abstract and publisher metadata, to a 156-page review published in 1984 that organized taxonomy, phytochemistry, ethnobotany, and pharmacology across the genus Tabernaemontana. It linked scientific names, synonyms, plant parts, alkaloids, traditional uses, and pharmacology of crude extracts and isolated constituents that had previously been scattered, and later sananga literature repeatedly cites it as a foundational reference.
The subject, however, was the whole genus Tabernaemontana, estimated to contain approximately 100 species, not T. sananho alone. The review concerns ethnobotanical plant uses, chemical constituents, crude extracts, and isolated alkaloids; it was not a clinical trial administering sananga eye drops to people. It did not establish clinical efficacy of sananga eye drops in humans.
Study design
The authors taxonomically reassessed as far as possible names for Tabernaemontana appearing in chemical and ethnobotanical literature and produced lists of accepted species and synonyms. Classification of the genus was under revision at the time, and they estimated that reintegrating several separated genera would leave approximately 100 accepted species.
For phytochemistry, the authors organized biosynthesis and classification of indole alkaloids reported from the genus and problems in determining stereochemistry. To make information accessible, they indexed alkaloids alphabetically by name, by molecular weight, and by biosynthetic classification, relating them to known species and plant parts. Tables of non-alkaloid constituents were also included. Ethnobotany was reviewed by species, while pharmacology reports on crude extracts and individual alkaloids were assembled and evaluated.
This was a large synthesis of published material, but as a 1984 review it differs from a present-day systematic review with PRISMA-style preregistration, database search strings, dual screening, and risk-of-bias assessment.
Content
An important role of the paper was reducing confusion from reports of the same plant under different scientific names and creating a framework for tracing “which compound was reported from which part of which species.” If taxonomic names are incorrectly linked, chemistry or traditional use from another species can be assigned to the wrong plant, making classification verification necessary even before pharmacology.
The phytochemistry section systematizes the diverse indole alkaloids characteristic of the genus. Listing not only compound names but biosynthetic classification, molecular weight, stereochemistry, species, and plant part created a foundation for later identification and comparison. Information on coronaridine, ibogamine, hydroxycoronaridine, heyneanine, voacangine, and other compounds cited by later T. sananho reviews appears as part of this genus-wide chemical map.
The ethnobotany and pharmacology sections gather regional use records and activities reported for crude extracts or isolated constituents. The units of evidence must remain distinct. Activity from an isolated alkaloid of another Tabernaemontana species is not activity of the whole T. sananho plant or of sananga eye drops. A traditional record of use in the eye does not become a human ophthalmic trial of a standardized product.
Results
The review’s achievement was organizing genus taxonomy and synonyms, making numerous alkaloids cross-referenceable through multiple indexes, and integrating non-alkaloid constituents, ethnobotany, and pharmacology reports on crude extracts and isolated compounds into 1 reference. The PubMed abstract states that the authors did more than list reports and comprehensively evaluated the ethnobotanical and pharmacologic information.
What the review provides is not a clinical effect size. The cited reports differ in period, species, plant part, extraction method, animal species, and outcome. The presence of diverse candidate bioactivities across the genus supplies hypotheses and a foundation for classification and chemistry; it does not show that a particular sananga product benefits the human eye or body.
Limitations
More than 40 years have passed since publication, so the review does not contain subsequent taxonomic revisions, analytical techniques, new compounds, or safety reports. Older studies may differ from current standards in botanical identification, specimen information, extract standardization, statistical analysis, and completeness of reporting. The authors themselves discuss problems in stereochemical assignment of alkaloids.
Genus-level evidence is broader than species-level inference, and information about T. sananho is broader than information about an aqueous eye preparation. Effects from another species, another plant part, a crude extract, or an isolated constituent cannot be combined into “the effect of sananga.” No randomized trial, comparative trial, dose-finding study, or safety trial of sananga eye drops in humans was included.
Safety
The large number of alkaloids and pharmacologic activities in the review suggests that exposure to plants in the genus may not involve a simple action of 1 constituent. Misidentification of species, differences in plant part and extraction, and variation in constituent concentration further complicate safety assessment. The review did not assess sterility, concentration, ocular irritation, or corneal toxicity of eye products now in circulation.
Records of isolated constituents or animal experiments do not establish a human safety range, contraindications, or interactions. This resource gives no preparation method, ophthalmic procedure, or amount and does not replace medical diagnosis or treatment.
